HMEA is currently seeking a full time (40 hour) Support Broker/ Case Manager to work with families who have children under the age of 9 and are enrolled in the Autism Waiver Program.

General Responsibilites:
Provide support as needed for families enrolled in the Autism Waiver Program.
Assist with Autism Resource Center with activities as needed.

Minimum Requirements:

• Bachelor’s Degree preferred, in related field and 3 years working experience with young children with ASD and their families, in lieu of BA, an Associates degree in related field and 5 years working experience*
• Must have Autism experience, or experience working with families*

Valid Driver’s license
Acceptable driving status, as determined by HMEA insurance Company
Bilingual (Spanish/English) Preferred

ESSENTIAL FUNCTIONS/RESPONSIBILITIES*
Sole responsibility of the Waiver Broker is to provide services to adults and children enrolled in the program
Must be able to work flexible hours in order to accommodate families
Must attend all meetings with Clinical Managers and other relevant DDS staff to review case loads
Assess capacity and strengths of families.
Assist families with developing an Autism Support Plan.
Assist families with identifying and hiring staff.
Broker services and supports from community agency resources.
Collaborate with other agencies to develop resources and create solutions.
Manage and document all progress notes and waiver budgets, in a timely manner, in accordance to Autism Waiver Program Guidelines.

First half at 30 days, Second half after completion of 90 days. Conditions apply
