Job description

This position is part of a multidisciplinary team that provides field based psychosocial rehabilitative services to individuals with severe and persistent mental illness. Other services and coordination of services include: medication training and supports, supported housing and employment services, co-occurring psychiatric and substance use disorders, and after hours crisis response and intervention.

The ACT Team Field RN provides evidenced based services to assist individuals in maintaining or improving their ability to function in the community and work towards their recovery goals.

Additional Duties Include:

  • Provide Service Coordination and Outreach & Engagement Activities to individuals on the ACT team including completion of needs assessment, individualized recovery planning, referral to appropriate services, monitoring of needs, and community engagement.
  • Provide crisis services and problem-solving assistance to persons on caseload

Requirements:

  • Must be licensed as a Registered Nurse in the State of New York.
  • Must have at least 1 year of Mental/Behavioral Health Nursing experience.
  • Have the knowledge and experience in providing rehabilitative services to individuals with mental illness.
  • Ability to write clearly and professionally
  • Have the ability to think clearly and logically, in order to deal with potentially dangerous and/or emotional situations.
  • Working knowledge of Behavioral Health medications (antipsychotics, antidepressants, psychotropic agents, and mood stabilizers, and anticonvulsants)
