The Assertive Community Treatment (ACT) is an intensive and highly integrated approach for community mental health service delivery. ACT programs serve individuals whose symptoms of mental illness lead to serious functioning difficulties in several major areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness. The ACT Therapist is assigned to individuals with severe and persistent mental illness, typically with a history of lengthy or frequent hospitalizations and/or a need for intensive community supports provided on an outreach basis. An important aspect of the ACT Therapist role includes serving as the treatment team chair and coordinator of case management activities. Under this model all ACT team members share in the responsibility and welfare of consumers who are assigned to the program. The level of intensity of service provided to each individual will be a function of the Person Centered Planning process and PCP treatment plan. Services relating to the case management support plan will be provided in a direct outreach mode with the goal being to maintain the individual in the community and reduce hospital recidivism. ACT is a 24-hour service; team members are required to provide scheduled day and night time services as identified by individual need. This position reports to the ACT Program Supervisor.

Education, Training and Licensing Credentials:
Masters Level (Pay Grade 9)
Master's Degree in Social Work, Psychology, or Counseling with State of Michigan Licensure.
Masters level such as
- Licensed Masters Social Work
- Limited License Psychology
- Licensed Professional Counselor
Experience:
One year paid work experience within the specific consumer population in which the position holds responsibility (adults with mental illness (QMHP*), adults with developmental disability (QIDP**), integrated care, assertive community treatment) required.
- QMHP - Individual with specialized training or one year experience in treating or working with a person who has mental illness as defined under the Michigan PIHP/CMSP Provider Qualifications per Medicaid Services.
** QIDP - Qualified Intellectual Disability Professional (QIDP) - Individual with specialized training or one year experience in treating or working with a person who has intellectual disability; and is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, registered dietician, therapeutic recreation specialist, or a licensed or limited-licensed professional counselor.
One year experience with the chronic mentally ill and vocational education preferred. Willingness to receive SUD training and obtain certification (CAADC, CADC, CCJP, CCDP, or CCDP-D).
Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served. Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individuals status; interpret the appropriate information needed to identify each individuals requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group (adult and geriatric).
