Part Time: 24 hrs/week – two 12-hr shifts per week to include every other weekend
Job Description:
- Admits patients to the hospice program and completes an initial comprehensive assessment to develop a plan of care based on patient/family needs and goals
- Educates patients and family members on end of life issues, self-care, hospice services and ensures consents and payer forms are completed
- Acts as a patient advocate. Provides support, information and direct care.
- Responsible for implementing and evaluating a patient plan of care utilizing ANA (American Nurses Association) Standards and the Nursing Process.
- Utilizes equipment and technology skillfully and is technically competent in nursing procedures.
- Oversees and/or participates in safe medication handling and practices
- Maintains appropriate licenses and adheres to continuing education obligations
Requirements:
- Licensed and currently registered to practice as a Registered Nurse in the state of Maryland, Delaware or other compact States
- Covid-19 vaccination required
- Must be able to lift and carry up to fifty (50) pounds with or without the use of assistive devices
