POSITION SUMMARY:
This is a professional-level position assisting adults who are elderly and/or disabled and their families who are dealing with varied issues and concerns related to aging, disabilities, and long-term care. Employee assists customers in identifying needs, determines program eligibility through assessments and functional screens, and links customers to community services and resources so that they may live with dignity, respect, health, and purpose. Position will work closely with community partners to develop collaborative relationships to ensure proper referrals and work together to serve older adults and adults with disabilities.

QUALIFICATIONS:
Bachelor's degree from an approved college/university, in Social Work, Nursing, or closely related field and one year of related work experience. Work experience with frail elderly or disabled adults in an outreach, social work, case manager, or counseling role desirable.
Necessary special qualifications: Ability to obtain Functional Screener certification by successfully completing required exams within 3 months of hire. Also required is a driver's license and a driving record that meets County standards; OR ability to obtain reliable transportation as needed.
EXAMPLES OF WORK PERFORMED:
- Provides information to older and disabled adults and their families on resources and options that are available to maintain health and independence.
- Communicates with the public concerning issues for target populations (elderly and disabled adults) and their families; provides information on resources, services, and long-term care options available to meet their needs.
- Completes outreach to community partners, including but not limited to schools, nursing homes and providers with a primary focus on developing strong collaborative relationships which remove barriers to successful referrals.
- Conducts long-term care functional screen assessments to determine functional eligibility for community-based long-term care programs which involves reviewing medical records.
- Coordinates enrollment into community-based long-term care programs by ensuring eligibility criteria are met and completing unbiased enrollment counseling.
- Provides support with Medicaid applications for eligibility determination for long term care programs
- Completes long-term care disenrollment counseling and program change counseling as well as coordinates this process.
- Completes required documentation in the program database and state functional screen website in accordance with policies and procedures.
- Understands and seeks out new or additional resources that are available for older adults and adults with disabilities.
- Develops and expands job-related skills and knowledge by attending trainings and reviewing information related to work practices
- Completes Meals on Wheels assessments
- Participates on agency committees and community focus groups as requested.
- Maintains regular and predictable attendance; works extra hours as required.
KNOWLEDGE, SKILLS & ABILITIES:
- Ability to work effectively with adults with disabilities and older adults.
- Ability to conduct assessments of clients' needs through telephone interviews, office interviews, and other arrangements as required.
- Ability to develop excellent rapport and work effectively with clients, families and professionals. Strong collaboration skills with the ability to gain cooperation from staff and community partners.
- Ability to be open minded and receptive to different points of view including supporting culturally competent and strength-based practices.
- Ability to listen and respond in a nonjudgmental, respectful manner to the unique circumstance and needs.
- Commitment to the mission of the ADRC-CW; ability to represent the organization’s work and mission to the public.
- Skill in interviewing and counseling older adults and adults with disabilities whether meeting as an individual, group or family dynamic.
- Knowledge of community resources and services that would assist older adults and adults with disabilities.
- Knowledge of and commitment to adopting a trauma-informed approach to practice.
- Ability to be resourceful and creative in identifying possible solutions to support these in the workplace.
- Ability to maintain confidentiality.
- Ability to communicate clearly and effectively with individuals and in front of groups- in both written and verbal formats.
- Ability to be organized, work under pressure and meet deadlines.
- Ability to keep and maintain clear, accurate, timely records.
- Ability to contribute to a positive work culture that fosters excellent customer service and teamwork.
- Skill in using a computer for email, calendaring, basic word processing and maintaining computerized databases.
COMPENSATION:
Starting at $28.22/hour + Marathon County benefits
