Compassionate Care at Home AFL is a program that helps individuals 18 or older remain living at home instead of relying on long term care such as assisted living or nursing homes. This program enables family members or friends acting as a primary caregiver, to get paid to care for a loved one in their own home.

Compassionate Care at Home understands the sacrifice being made by family and friends to care for a loved one at home. Through AFL, you can receive direct compensation for your hard work while keeping your loved one safe in the comfort of their own home.

Eligibility:

Client must be 18+ and currently receiving benefits under Title 19 (Medicaid) in Connecticut.

Caregiver must live in the same house as the client in need of care.

Client must need assistance with daily living: bathing, dressing, transferring, medication, feeding, personal care, walking, supervision for a cognitive impairment (ex. Alzheimer’s or Dementia).

Who can be an AFL Caregiver?

The uniqueness of the AFL program is that clients can choose their own caregiver. However, please note the caregiver must:

* Live in the same house as the client in need of care.

* Cannot be a legally liable relative such as a legal spouse or POA.

Process:

Once Compassionate Care at Home receives approval to begin services, you will be contacted by one of our trained professionals. They will set up an appointment to conduct an initial assessment in the home of the client.

After the initial assessment is completed, the level of care will be determined, and the program can officially start! The caregiver will be trained and receive ongoing 24/7 support from our Compassionate Care at Home team. Throughout the program, there will be follow-ups and bi-monthly nursing visits.

Payment

Compassionate Care at Home caregivers are compensated based on the level of care that is determined by the initial assessment. Compassionate understands that caregiving is a labor of love, that is why Compassionate provides the best rates in the state. Compassionate AFL Caregivers receive bi-weekly tax-free payments.
