After Hours SUD Treatment Coordinator Purpose: This position is responsible for screening, enrolling and providing group and individual substance use disorder treatment for eligible program participants, which includes folks living with HIV and members of the LGBT+ community. The Coordinator will schedule, arrange, and meet with folks during non-traditional hours, in order to meet the needs of individuals who may be working or otherwise unable to attend treatment during traditional hours.

The Coordinator will also assure that the program remains in compliance with all federal, state and local rules and regulations, and achieves and exceeds the goals for the program. All employees are responsible for maintaining a trauma-informed, client-centered, harm-reduction approach to services, and for developing collegial working relationships with co-workers and community partners.

Major Areas of Responsibility:

1. Review, screen and document client referrals for the After Hours program from internal staff and external community partners.

2. Serve 40 clients over the course of the grant period (ends June 30, 2023).

3. Complete intake and assessment process for eligible clients in the program(s).

4. Work with clients to develop a care plan with measurable goals related to engagement and retention in SUD treatment and harm reduction.

5. Document all activities in Simple Practice weekly.

6. Attend required Mecklenburg County ABC Board meetings, including site visits, audits, and meetings with staff, if requested by the funder.

7. Interact with medical providers to document progress towards treatment plan goals.

8. Attend supervision with Supervisor and Clinical Case Conferencing if needed.

Agency Requirements

1. Comply with all requirements, rules, regulations and goals of the grant programs.

2. Maintain and update all client file information in a timely manner and as required in accordance with policy.

3. Comply with terms and conditions, including recordkeeping and reports for program monitoring and evaluation purposes of carrying out the program in an effective and efficient manner.

4. Interact with HIV case managers, Housing providers, and others involved in assisting clients.

5. Assure PLWH are linked to medical care and treatment.

6. Develop and implement mechanisms to always assure that clients' confidentiality is protected.

7. Other duties as assigned.

Qualifications:

  • The After Hours Program Coordinator must have core competencies that include having a strong skill-set for outreach and being able to engage effectively with clients. The person will have experience with assisting clients in developing and/or enhancing life skills that will support their ability to maintain engagement in SUD treatment. The After-Hours Program Coordinator will possess a strong knowledge of community resources and the ability to assist clients in accessing them, as needed. Finally, the Program Coordinator will have the proper knowledge, attitude, and expertise to work with individuals who are living with HIV and LGBT+ individuals.
  • Outstanding interpersonal skills. Suitable demeanor to work with PLWH and LGBT+ individuals, build rapport, maintain engagement, and conduct outreach to recruit clients as needed.
  • Belief in and ability to adhere to the principles of harm reduction, healing-centered, trauma-informed, and client-focused approaches to both one-on-one and group services.
  • Four-year human services or related degree, SA Certification, and 1 year of experience providing SUD services in group and individual settings.
  • Proficient in working with a personal computer (Word and Excel). Ability to develop and maintain tracking record of work activities. Ability to work within, and adhere to program regulations. Ability to provide services to individuals with diverse backgrounds. Excellent oral and written communication skills. Ability to interact professionally with staff, board members, committees, agencies, medical providers, and the general public. Ability to exercise good judgment, handle confidential issues, and work with the agency's other team members.
  • Possess evaluation skills for assessing community needs; demonstrated ability to work independently as well as part of a team; excellent writing, reporting, speaking and listening skills; ability to work well with committees and community agencies.
  • Possess valid driver’s license and insurance, and have reliable transportation.
