Primary Care – Nurse Practitioner
Excellent opportunity for an Family Medicine APC to practice in an outpatient setting.

Summary:
- Full-time, 1.0 FTE, is 32 patient contact hours, 8 administrative hours
- Collaborative team model that includes medical assistants, nurses, care coordinators, social workers, and population health
- Full support to perform primary-care based procedures, such as joint injections, Nexplanon, minor dermatologic procedures

- 3-5 years experience required
Requirements:
- Graduate of an accredited Nurse Practitioner program and appropriate licensure and certification
- Team oriented with excellent communication skills
- Comfortable seeing all visit types/ages efficiently in a fast-paced setting
