SUMMARY OF POSITION

The Integrated Care Social Worker will focus primarily on preventative interventions to our chronic disease patients who can also suffer from behavioral health and substance abuse issues. This position will provide case management, a limited amount of individual counseling, and group wellness and health education

ESSENTIAL DUTIES

• Participate in health education program development and implementation.
• Facilitate group health education sessions, utilizing various modalities: process-oriented, curriculum-based, and/or psycho-educational, with topics focusing on and related to chronic disease management.
• Utilize cultural competency, active listening, and motivational interviewing skills to assist clients in reaching their treatment goals.
• Maintain timely administrative, assessment, and clinical records and documentation related to client care.
• Conduct case management.
• Conduct limited individual counseling services.
• Collaborate and closely coordinate with clients, as well as case managers, including other providers (eg. physician, psychiatrist, psychologist, social worker) in treatment planning and care and goal development.
• Partake in and facilitate various client outreach activities to further support engagement and retention in behavioral health services, as requested.
• Participate in various educational & training workshops relating to mental health, physical health, and/or substance use issues and psycho-educational topics and may facilitate such meetings for clients and other professionals & program staff, as deemed necessary.
• Attends staff meetings.
• Assumes other duties and tasks as assigned by the direct supervisor and department director.

Pay: $20.00 - $41.00 per hour
