Job Title: Social Worker - ACSW - Telephonic Case Management

Type: Full-Time, with Benefits available

Location: San Fernando Valley, CA

Responsibilities:
-Evaluate clients’ psycho-social needs;
-Evaluate clients’ family/social situation to support a treatment plan;
-Participate in on-going counseling
-Make recommendations to the healthcare team for appropriate psycho-social support
-Utilize community resources to assist clients’ psycho-social and homeless needs;
-Document history, care plans and prepare reports as needed.

Experience Requirements:
1 years of experience as a Social Worker (ACSW);

Education Requirements:
Graduation from an accredited university or college with a Masters Degree in Social Work.

Qualifications:
-Current CA ACSW license

-Current AHA-approved CPR card

Skills:
-Skills in critical thinking, problem-solving, logic and reasoning, judgment, time management techniques and data entry;
-Ability to make improvements or take corrective action;
-Observe, receive, and otherwise obtain information from all relevant sources;
-Organize, plan, and prioritize work;
-Communicate with people outside the organization, represent the organization to patients, clients and the public;
-Deal calmly and effectively in high stress situations;
-Develop constructive and cooperative working relationships with others, and maintain them over time;
-Work within a team as well as independently;
-Speak, read, and write effectively as appropriate for the needs of the audience while demonstrating the proper use of grammar.
