Overland Park Center is actively seeking experienced Behavioral Health LPN/RN's to work for our skilled nursing community in Overland Park KS.

Duties of Behavioral Health LPN/RN :

• Collecting required information from new residents to be admitted
• Recording health details of residents, including vitals and temperature
• Administering medications and injections to residents as needed
• Treating and dressing wounds and bedsores as needed
• Helps residents get dressed and take care of personal hygiene
• Monitors resident's food and liquid intake and output
• Provide the necessary care, observation/assessment, teaching/training activities, management and evaluation of a patient care plan, or direct patient care of a diagnosed psychiatric behavioral/cognitive interventions.
• Performs client admissions, resumption of care, follow-up using OASIS data collection in a timely, legible, accurate manner
• Responsible for monitoring and managing core measurements pertinent to the department to assure compliance with the operating plan.
• Identifies, documents and reports signs and symptoms of altered health status to physician and Clinical Team Manager.
• Obtains appropriate orders to increase/decrease visit frequency including medical orders to alter treatment plan.
• Administers meds and treatment according to Plan of Treatment.
• Documents client records according to policy.
• Completes clinical notes, Plan of Care and medication records in a timely, legible and accurate manner.
• Demonstrates safety precautions in compliance with OSHA, JCAHO/CHAP, and Federal and State standards.

Requirements of Behavioral Health LPN/RN:

• Must be able to work as a team member
• Valid KS State LPN/RN license
• Must be in good standing with State Registry
• Must have 1 year experience in behavioral health for adults/geriatrics
• Full time/ Part time position
• Must be able to work 12hour shifts
