Pathfinders Recovery Center is a substance abuse treatment facility in Scottsdale Arizona. Care Coordinators have a variety of responsibilities many of which are direct care to the patients we serve. Such tasks include medication monitoring, transporting clients in company vehicles, ensuring safety of clients and working with the interdisciplinary team to carry out treatment plans for each individual. Other responsibilities often look like taking and recording accurate vital signs, record clinical documentation in the EHR, and deescalation and MI to help move patients through the stages of change.
