Position summary:

  • Help Patients navigate clinical pathways, connecting to resources internal and external to MaineHealth
  • Identify and address barriers to care, in order to transition patients to primary care and self-care
  • Work closely with teams at Maine Behavioral Health and Certified Community Behavioral Health Clinics (CCBHC) to ensure coordination and flow of services.
  • Data entry to ensure that CCBHC reporting needs are met.
  • 40 hour work week, office open M-F, varied hours between 7AM-7PM.

What we are looking for:

  • A Bachelor’s degree in Social work, or related field
  • Individuals who are positive, outgoing and customer relationship orientated.
  • Individuals comfortable with technology and data
  • Excellent communication skills
  • Ability to be self-directed in work, whilst maintaining a team focus.
  • Comfortable advocating for patients with all levels of providers (including those with higher credentials)
Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: Bachelor in social work or related field preferred
  • License/Certifications: None required
  • Experience: Minimum of two years’ experience working with behavioral health consumers and families. Experience working in team based settings and collaborating with multiple providers preferred.
  • Demonstrated competency in applying evidence based knowledge, theory, practices, principles, and processes
  • Compassionate, articulate and patient.
  • Knowledgeable about the breadth and accessibility of community resources.
  • Strong communication, interpersonal and organizational skills.
  • Ability to build and maintain solid interdepartmental relationships.
  • Strong problem solving, critical thinking, and advocacy skills.
  • Abillity to engage with external stakeholders, participate in community coalitions, and partner with police/corrections if needed
  • Ability to identify potential crisis situations involving patients and/or families and to initiate appropriate action.
