Case Manager Job Description
This position reports to: MVP Executive Leadership

Internally, this position teams with: Meridian Medical Services’ team members.
Externally, this position interacts with: MVP patients/caregivers and professional vendors/contacts
Job Purpose
The Case Manager is responsible for the successful triage and execution of patient concerns (by phone call, fax, or email) as well as carrying out any/all orders from MVP providers regarding patient care, and successfully population managing the assigned patients through Chronic Care Management and patient/provider satisfaction.
Overview
General Duties
· Performing Case Management outreach to populations, ensuring needs are met, orders are fulfilled, and conducting Chronic Care Management.
Tasks and Responsibilities
· Discussing patients’ care with patients, professional associates and caregivers to ensure that all patient needs are being met and patients are remaining healthy in their home environment
· Getting messages and orders to providers as result of clinical discussion and carrying out orders
· Actively triaging urgent clinical needs to the provider for intervention and treatment
· Communicating effectively and efficiently with everyone involved in patients’ care to ensure all parties have the most up to date information available for optimal patient care
· Keeping accurate clinical and time keeping records for multiple logs where patient information is stored and managed
· Answering phones and making calls in positive and upbeat manner and directing to appropriate staff member and returning messages that have been left throughout the duration of the day
· Being included in on-call rotation for clinical concerns that arise outside of normal business hours
· Scheduling patient routes based on resources available and within expectation parameters and needs/desires of field clinicians
· Conducting, completing and maintaining complete and compliant records for Chronic Care Management for all assigned patients
· Remaining HIPAA compliant
Pay, Benefits and Expectations
· The position is full-time at an hourly rate with the expectation that the applicant is available Monday-Friday from approximately 8:00am– 5:00pm
o 30-minute lunch period
· Competitive/negotiable hourly pay rate starting at $17.00/hr
· Benefits available after probationary ~90-day period from date of hire:

Physical Demands
· When in the office, there are little physical demands required, but the Patient Care Coordinator will likely be on his/her feet for ~50% of the day
· Expected travel is 5% or less
Qualifications
Education
· EMT, LPN Degree, Medical Assistant Degree or Bachelors’ Degree required
o Alternate or additional clinical education will be considered
· At least 1 year of social work or case manager experience preferred
· At least 1 year of experience in a medical office required
· At least 1 year of customer service experience required
Knowledge, Skills and Characteristics
· Must be well-organized, open-minded, and flexible
· He/she must be able to communicate well with others, work as a team, and lead others by example and direction
· Excellent written and oral skills are necessary and applicant must have professional phone and in-person etiquette
· Strong computer skills and EHR experience required
· Ability to multi-task and remain organized and positive throughout is a requirement
· Applicant must be customer service focused and have a strong attention to detail and positive, upbeat personality
· General knowledge and comfort with medical terminology and medications is required
· Comfort and experience with Microsoft Office is preferred
· Applicant must be dependable and accountable for actions and responsibilities
