Job description

Our organization is seeking an experienced medical Case Manager to join our team. The successful candidate will be responsible for providing comprehensive case management services to clients, attorneys, and providers iincluding implementation, coordination, monitoring and evaluation of services. The Case Manager will also be responsible for developing and maintaining relationships with clients and other service providers in order to ensure the most effective delivery of services. The ideal candidate will have a strong background in personal injury care or a related field and excellent communication skills.

Responsibilities:

• Assess clients’ needs and develop individualized service plans.
• Provide case management services to clients, including referrals to providers, diagnostic centers
• Monitor client progress and provide ongoing support.
• Collaborate with other professionals to ensure client needs are met.
• Maintain accurate records of client progress and services provided.

Job Type: Full-time

Pay: $60,000.00 - $65,000.00 per year

• 8 hour shift
• Monday to Friday
