Sojourner Truth House is a ministry of hope and help for homeless and at-risk women and their children sponsored by the Poor Handmaids of Jesus Christ (PHJC). Through hospitality, advocacy, integrative services and collaboration, participants improve their quality of life and that of the community in a safe, trusting environment.

Job purpose
Performs case management services for homeless clients. Researches and networks with community resources for referral purposes. Assists clients and families in the alleviation or resolution of social, financial, and emotional problems related to housing, health, substance abuse and sustainability. Assists supportive housing clients to maintain their housing and increase their self-sufficiency through linkages to resources and the attainment of established goals. In addition to maintaining a caseload, assists management with all audits and the activities of case managers.

- Possesses a working knowledge and commitment of the values that the Poor Handmaids of Jesus Christ hold dear
- Adheres to the Sojourner Truth House code of ethics and HIPPA Regulations for confidentiality
- Establishes and maintains positive community relations
- Maintains cooperative working relationships to promote the ministry and networks with other professionals and community agencies
- Provides quality service and professional competence
- Provides direct services to clients
- Represents the ministry within the community when appropriate or assigned
- Demonstrates sensitivity to the cultural needs of a diverse client population
- Supports ministry initiatives and the goals of the ministry’s strategic plan
Duties
- Develops initial service plan specific and tailored to the client needs identified during initial intake assessment/screening process.
- Sets up case management plans with clients as therapeutic intervention .
- Verifies client information and requests necessary documentation for client folder.
- All progress notes must be completed in SOAP format and signed and dated with appropriate credentials
- Enters initial intake data in Client Track within twenty-four hours of that intake.
- Completes chronological progress notes within 48 hours of contact with ongoing client or with others involved in client case
- Maintains and updates client code of conduct and program expectations
- Participates in monthly client staffing
- Completes referral documentation and informed consent for supportive services.
- Assists clients with enrolling their children in school as needed
- Attends court to support/advocate for client as appropriate
- Provides transportation services for clients as needed
- Conducts two monthly contacts including one face to face contact per client in Gateway program
- Revises or updates services plans every six months or based upon the client level of need.
- Assists no-income clients to identify and obtain entitlement(s) within six months of admission to the housing program.
- Routinely assesses strengths and barriers to income, housing stability and self-sufficiency (including health, substance use and mental health issues), and provides appropriate referrals to supportive services in a timely manner.
- Assists with mediating any housing issues that might arise.
- Provides advocacy, support, and linkages/referrals to community resources
- Communicates with service providers regarding client needs and follow-up. Conducts follow-up on all referrals and documents referral outcomes.
- Completes monthly statistical report and submits to immediate supervisor by tenth business day of each month
- Participates in all mandatory external and internal meetings or trainings.
- Assists with or facilitates therapeutic group activities provided to clients
- Schedules and assists with client drug screens
- Resolves crisis situations appropriately
- Demonstrates a collaborative work style with co-workers, supervisors, volunteers, interns, and others who help the ministry.

Qualifications
- Bachelor’s degree in Social Work or comparable degree in Human Services
- Experience working in a multi-cultural environment for a not-for-profit organization providing direct service to the poor
- One or more years’ experience in Case Management or Human Services or related field
- Experience working with individual with substance or behavioral problems (prefer

- Lifting, pushing, pulling, and carrying items up to approximately 40 lbs.
- Traveling may be required to perform assigned tasks; ability to operate a motor vehicle. Valid driver’s license and auto insurance required
