Case Manager

Stress Care of NJ is an outpatient mental health and substance Abuse treatment center licensed with the NJ Dept of Human Services and JCAHO accredited for individual and group work to help clients primarily in from Monmouth, Ocean, and Middlesex counties.

The Case Manager will provide direct community services and assistance in the implementation of the Care Plan for each client and/or family enrolled in Stress Care's Behavioral Health Home.

Responsibilities and Duties

  • Ensure engagement and retention by means of client outreach
  • Complete initial screening and orientation for new clients
  • Perform risk assessment and safety planning
  • Assist recipient with application of services, arrange transportation, and escort to appointments as necessary
  • Provide coaching and health promotion activities to recipients using individual, family and group forums in accordance with best practice guidelines of Chronic Illness Self Management model
  • Interface with medical professionals and others on behalf for the recipient as appropriate
  • Maintain communication with client, family, and community providers to problem solve barriers
  • Submit Quality Assurance data in accordance with CMS required quality measures
  • Participate in weekly supervision and staff meetings

Requirements

  • Bachelor's Degree in psychology or social work, only
  • 1 year case management experience
