We have an opening for a full-time Caregiver Case Manager. Responsibilities include providing support to caregivers of an adult over 60 or to caregivers of a relative child 18 or younger, supportive counseling, problem solving, mentoring. Must have reliable transportation and be able to maintain a flexible work schedule to provide service in multiple counties in central IL. Bachelor degree in human service field or RN / LPN with 1 year experience required. Counseling experience and experience working with seniors preferred.
