The Case Worker will support individuals and families within Vineyard Community Center and Vineyard Columbus in providing case management services. Are you passionate about social justice and love to see people get the care they need? If yes, then read on to see if you’re a fit for this position.

What you’ll do:
  • Assess and connect the client with services and resources to meet their needs
  • Support various ministries and programs within the organization to engage with clients and provide follow up and holistic care for the individual(s)
  • Serve as an advocate between the client and various organizations
  • Provide flexibility and support for the changing needs of the ministry
  • Maintain systems for tracking and reporting progress of activities
What you bring :
  • Bachelor’s degree or equivalent in social work, criminal justice, human services, or relevant field
  • A high proficiency in Microsoft Office software and database use
  • Strong interpersonal skills
  • Minimum of 2+ years of professional experience in advocacy, case management, social services or related field; or a combination of education and experience
  • LSW preferred
  • Bilingual (Spanish speaking) preferred


  • This is a grant-funded position with funding available for up to 2 years.
