Job Title: Certified Home Health Aide

Department: Quality Care – Hospice

About the Role:

ApolloMed is looking for a Certified Home Health Aide - Hospice to work at our Alhambra, CA location to provide quality care for patients in the comfort of their home.

What You'll Do:

• 
  Provide essential personal care assistance to the patient in accordance with the plan of care and will be supervised by a registered nurse in accordance with applicable laws and regulations
• Assist patients with activities of daily living and performs light household services that are essential to the patient's well-being
• Provides emotional support and compassionate care to the patient and family unit
• Reports any changes in the patient's condition or family situation immediately by calling the Hospice office and notifying the RN

Minimum qualifications:

• 
  Active and unrestricted Certified Home Health Aide in the state of California
• Ability to drive to client sites in the state of California
• High school diploma or GED
• One (1) year of experience as an Aide

• Ability to respond appropriately to crises outside of a hospital setting
• Understands philosophy of hospice concept and needs of the terminally ill

Environmental Job Requirements and Working Conditions:

• 
  This position is in office. The office is located at: 1668 S Garfield Ave, Alhambra, CA 91801.

• You will be required to travel to patient homes but will be reimbursed at the IRS mileage rate. This position's schedule is typically M-F from 9am to 5pm but will also have potential to work outside of those hours and during weekends if there is patient need.
  
• The Target pay range for this role is $16.50 - $28.50. This salary range represents national target range for this role.
