Summary:

The Outpatient Therapist shall provide a range of individual, family and group counseling services to assigned individuals from a variety of diagnostic categories and needs (mental illness, severe emotional disturbance, co-occurring mild intellectual/developmental disabilities and co-occurring disorders). The Outpatient Therapist (LLMSW, LMSW, LLPC, LPC, TLLP, LLP) will provide the initial assessment for all MI Health Link enrollees as well as the individual therapy to these individuals. Linking/coordinating with other community providers may be necessary. Least restrictive environment of care will be promoted for all individuals. All services will be provided within a "treatment team" concept. The Outpatient Therapist will be directly report to the Children Services Supervisor.


Duties:


Provide face to face services (individual and/or families) as specified in the Person Centered Plan. Completion of additional PCP documents, including safety and crisis plans and advance directives.

Provide services using evidence based or promising practice treatment modalities (Cognitive Behavioral Therapy, Motivational Interviewing, recovery oriented and trauma informed).

Documentation completed is timely, strengths based, acknowledges any integrated health activities, and measures the individuals/families progress and satisfaction with services, and includes services notes completed in the DAP model, periodic reviews, updated/annual assessments, updated treatment plans, diagnostic document, and transfers, as well as keeping the demographic information updated.

Provide crisis intervention to any individuals served, including immediate linking of any individuals in acute crisis who may require screening for crisis residential or inpatient psychiatric hospitalization to the Intake/Access/Emergency Services Unit (ESU).

Completion of BHTeds data at designated intervals, per MDHHS requirements.

Complete Level of Care and Outcome assessments at designated intervals, as applicable, to include the CAFAS and PECFAS.

Complete Action Notices after 30 days of no activity and at completion of therapy services, and complete Discharge summaries in the required time frame from time of issued Action Notices.

Complete Coordination with the Primary Care Physician and/or Qualified Health Plans.

Attend and participate in a variety of in-service trainings to maintain and improve licensure, job skills, including all agency-required in-service training and designated staff meetings.

Actively participate in scheduled supervision sessions with immediate supervisor, using feedback for professional development.

Maintain a minimum of 80% overall satisfaction as measured by designated consumer satisfaction rating scales (during designated measurement periods).

90% of consumers will demonstrate maintenance or decrease in PECFAS/CAFAS scores at time of annual review.

Act as liaison between CMH and other local agencies serving assigned individuals (MHDHHS, courts, schools, etc.).

Consult with staff psychiatrist and psychiatric nurse regarding consumer psychiatric and wellness needs

Provide direct consultation with family and relatives essential for individual progress, contingent upon the receipt of appropriate releases to disclose confidential information

Adhere to all policies and procedures of CMH, MDHHS, CARF, Medicaid, and other regulating bodies as may be applicable, including completion of 24 children’s specific mental health training each calendar year.


Education/Experience:


MA or M.S.W. Must be eligible for and maintain state licensure. (MH)

Minimum of two years work experience in related field preferred.


Knowledge, Skills, and Abilities:


Presents a welcoming manner to all customers and demonstrates engagement with individuals served, which adheres to the fidelity of a recovery model.

Lived experiences with diverse racial, ethnic, and cultural backgrounds, including experience with behavioral health issues is preferred. Must be supportive of culturally competent recovery-based practices and person-centered planning as a shared decision making process. Must support a trauma informed culture of safety to aid consumers in their recovery process.

Knowledge of advanced communication and empathic listening skills,

Crisis intervention skills

Excellent time management, personal organization, and decision-making skills

Knowledge of social service agencies and available resources

Ability to work with other staff in a team concept

Ability to accept supervision and complete assignments and duties accurately and in a timely manner


Working Conditions:


The Children’s Outpatient Clinician- Full Time is located at the Austin St. site and may be expected to attend various meetings and/or provide out-of-office services as needed.

Work pace and concentration required is constant to intermittent with an above average amount of detail often causing pressure or the feeling of pressure
