Position Description

The PSRB Clinical Case Manager works as a part of the Psychiatric Security Review Board clinical team providing community and clinic based interventions designed to support service recipients in obtaining their goals while providing supervision and monitoring per the individuals conditional release plan. Case Managers follow procedures, outcome goals, and reporting requirements as outlined by New Narrative’s policies and procedures. Services emphasize making use of natural community supports such as: DHS, SSA, family, friends, landlords, residential support staff, employers etc. Case Manager will be expected to go to individual’s home, as well as transport individuals to and from appointments and advocate for consumers.

Summary of Job Duties

  • Assist Mental Health Clinicians in fulfilling their duties including meeting with clients in their absence.
  • Assist participants in connecting to resources needed to achieve goals set in the individual service and support plan.
  • Support individuals in skills training as they reintegrate into the community from higher levels of care. Including leading skills training groups.
  • Coordinate and perform observed UDS’s for PSRB service participants
  • Monitor medication distribution and assist with prescriber visits.
  • Transport individuals to and from: DHS, SSA, MD, prescriber visits, therapy, etc.
  • Work proactively with individual to avoid use of crisis services.
  • Complete all required documentation
  • Observe hospital contact and service conclusion planning policy.
  • Minimize number of days spent using crisis/respite services intensive by remaining closely involved in service conclusion planning.

Qualifications

  • Qualified Mental Health Associate with a Bachelors Degree in social work or related field is required.
  • A minimum of one year’s experience in a human services field.
  • Demonstrate some understanding of psychiatric conditions and medications and their use, administrative rules and other applicable laws and regulations, and thorough understanding of acceptable clinical practices and guidelines.
  • Be comfortable spending unstructured one on one time with individuals who have been found guilty except for reasons of insanity for felony crimes, including sex offense and capital offense.
  • The ability to understand mental health assessments and implement initial individual service and support plan interventions.

Job Type: Full-time

Pay: $22.00 per hour

Schedule:

  • 8 hour shift
