Company Intro: Ocean Health Initiatives, Inc. (OHI) is a Federally Qualified Health Center (FQHC) dedicated to providing quality, accessible and comprehensive primary health care to the residents of Ocean and Monmouth County; regardless of economic status.

Our Health Center Locations: Brick, Freehold, Lakewood, Little Egg Harbor, Manahawkin, Manchester and Toms River; with our school-based Wellness Programs located within the Clifton Ave Grade School, Lakewood; and Lakewood High School.

OHI services include family and internal medicine, pediatrics, behavioral health, nutrition, OB/GYN women’s health, dental, family planning, specialty care, pharmacies, PrEP, and a STI clinic.

Position Summary

The clinical community healthcare provider is responsible for patient case management providing clinical services directly within OHI patient communities. This position will be responsible for new program growth and development and in-depth knowledge of organizational practice. Additionally, provide supervision of the outreach team on OHI’s mobile medical unit and coordinate all clinical activities and events.

Responsibilities

  • Identifies patients in risk categories to develop and implement appropriate plans of care with a strong ability to engage the underserved and disenfranchised patient population.
  • Assures quality clinical care by coordinating all services and collaborating with other OHI providers on plan of care.
  • Implements, monitors and evaluates the plan of care and its effectiveness on a regular basis.
  • Leads and motivate the multidisciplinary outreach team to achieve desired results.
  • Communicates regularly with relevant staff members and family members on patient's progress.
  • Advocates and provide COVID-19 testing and vaccinations.
  • Monitors social determinants of health (SDOH) of patients, and any identified with an unmet need associated with a determinant of health will be provided with education or resource connection.
  • Ability to provide all patient services including women’s health, well child visits, immunizations, and promote healthy lifestyles and health literacy
  • Develops and maintains effective working relationships with patients, families and with community resources, including medical, mental health, and substance abuse professionals, school officials, neighbors, and religious and youth organization representatives.
  • Maintains current knowledge of available formal services, resources, and supports in the local community.
  • Provides assessment to determine program eligibility, problem areas and areas of strength.
  • Develops, monitors, and reviews service plans and develops subsequent service plans as needed to facilitate progress
  • Works collaboratively with CMO and data analytics team to develop program metrics, track and trend needs to ensure program effectiveness, and provide to relevant committees for analysis.
  • Prepares and submits reports concerning case load status, budget requests, changes in service plans, and any unusual incidents that occur in the operation of the program.
  • Reports any critical incidents involving patients or staff to management.
  • Prepares proper documentation including case notes, assessments, service plans, outcome measures, narratives, and reports in a timely manner as defined by the program.
  • Implements feedback from supervisor.
  • Ensures compliance with all contractual, regulatory, program, and accrediting body standards.
  • Participates in multidisciplinary quality and service improvement teams as appropriate.
  • Participates in meetings, serves on committees and represents OHI in community outreach efforts as appropriate.
  • Is highly accountable for one's own productivity. In order to maintain transparency, all projects must be represented clearly on a project board in Monday.com. Responsible for ensuring the board is always updated and accurate and that information is accessible to those that need it.
  • Maintains a passing monthly scorecard threshold at 80%.
  • Adheres to corporate compliance policies.
  • Other duties as assigned.


Education/Experience/Licensure

  • Master degree in Nursing from an accredited School of Nursing is required.
  • Active State of New Jersey RN and Advanced Practice Nurse License is required.
  • Active State of New Jersey CDS and DEA License to prescribe and maintain controlled substances is required.
  • Certification by the New Jersey State Board of Nursing as an Advanced Practice Nurse is required.
  • BLS for providers through the American Heart Association is required.
  • Three to five years of experience at NP level in health care setting is required.
  • Must be flexible with providing services nights and weekends as needed.
  • Ability to find solutions to difficult human, technical, clinical, and/or administrative problems.
  • Ability to form working relationships with internal contacts as necessary to effectively advise, consult, or counsel to gain cooperation and acceptance of ideas or actions.
  • Ability to achieve results and meet deadlines without explicit instructions.
  • Proficiency in Microsoft Office 365 is required.
  • Strong communication and leadership skills and ability to influence professionals irrespective of reporting relationship is required.
  • Ability to build strong relationships with key partners and stakeholders across the organization.
