JOB SUMMARY: Responsible for establishing initial point of contact for individuals calling agency Access Center. Conduct telephonic screening to determine program eligibility, and provide information regarding rapid intake process. Provide referral information for community based services, as needed which may include crisis intervention counseling and referral. Serve as outpatient client liaison.

JOB RESPONSIBILITIES:

1. Coordinate Access Department staff coverage to ensure rapid response for all incoming calls.

2. Responsible to oversee all screening; and the initial clinical assessment of intake referrals received, and properly document the findings in the electronic medical record.

3. Acts as the initial point of contact for individuals, family and/or provider during the referral process.

4. Provide clinical triage for mental health; and substance abuse services as needed, including when appropriate making appropriate referrals for alternate levels of care.

5. On a case by case basis; will provide education and referral information to ensure linkage to community based services.

6. Evaluate inquiry for service, assess priority needs, and coordinate the scheduling of the Clinical Assessment.

7. Perform initial bio- psychosocial screenings as part of the agency rapid intake assessment process and identify initial individualized treatment recommendations.

8. Participate in consultations with Outpatient Director and other members of the interdisciplinary treatment team to facilitate the delivery of appropriate coordination of care.

9. Complete all documentation as required by agency and regulatory bodies.

10. Conduct initial financial screenings, verify benefits, and record insurance and other financial information.

11. Provide follow-up phone calls to clients missing appointments, and make attempts to clinically re-engage.

12. Complete administrative tasks related to departmental functions, and engage in inter- organizational care coordination.

13. Participate in outpatient staff meetings, in-services, and supervisory sessions as requested by Director.

14. Perform other duties and responsibilities to enhance ACCESS services as requested by Management.

15. Adhere to strict ethical boundaries and confidentiality required by VHS.

16. Adhere to requirements in General Skills Evaluation

17. Will perform other duties and responsibilities as requested by VHS management without giving up the right to the grievance process.

JOB QUALIFICATIONS:

EDUCATION: A Master’s Degree in Social Work or Counseling is required. Individual must be licensed or working towards licensure by the State of New Jersey as an LSW, LCSW, or LPC. Individual must have active license within one year of moving into role.

EXPERIENCE/KNOWLEDGE: Experience in people centered services, with minimum of 2 years in public related services. Have a working knowledge of severe mental illness, emotional and behavioral disorders, co-occurring and substance use disorders, a plus.

PHYSICAL: Visual and auditory acuity sufficient to evaluate and monitor client needs, and maintain accurate records.

Fine motor skills for legible written documentation and proficiency in use of computer. Ability to work required hours and fulfill job requirements.'
