Job Details - Clinical Liaison RN

• Act as a liaison between the hospital, the patient, and employer to determine eligibility for hospice services
• Create relationships with hospital staff to generate referrals
• Must be comfortable providing education to providers, patients, and their family members
• Communicate with other nurses throughout the process to facilitate referrals, new admissions, assessments, etc.
• Maintain patient confidentiality and demonstrate professionalism at all times

Clinical Liaison RN Requirements:

• Active and clean RN license in DC
• 3+ years of experience (hospice, home health, med surg, ER, ICU settings preferred)
• Reliable transportation and valid driver's license
• EMR proficient/tech savvy

Compensation + Benefits:

• Monday-Friday schedule
• Competitive salary: $80k-$100k+ (can increase and highly dependent on years of experience)
