Job Description
Implements, facilitates, and monitors clinical pharmacy initiatives as directed by management. Performs MUEs (Medication Usage Evaluations) and collaborates with medical, nursing, and pharmacy staff. Promotes safe, effective and appropriate use of medication. Adheres to required local, state and federal regulations and established pharmacy policy and procedures governing inpatient pharmacy services.
Job Responsibility
1. Collaborates with other healthcare professionals and patients to optimize medication therapy, as assigned, in accordance with established policy and procedures.
- Reviews patient charts to ensure safe, effective, and appropriate medication therapies
- Reviews medication orders for completeness, interactions, and incompatibilities/inconsistencies.
- Conducts medication reconciliation as needed to ensure no discrepancies in medication orders.
- Consults with prescriber for discrepancies and follows-up on issues to resolution.
- Monitors patients for Adverse Drug Events (ADEs) and efficacy of medication regimen.
- Documents clinical interventions and ADEs as per established policy and procedures.
- Educates patients on medication therapy to ensure understanding and compliance with prescribed regimen.
2. Assists in the implementation of the hospital�s policies and procedures regarding utilization of medications in collaboration with medical, pharmacy, and nursing staff
- Monitors medication utilization effectively and intervenes when necessary to ensure compliance.
- Keeps tracks of relevant metrics and data as related to policies and procedures.
- Maintains necessary and appropriate records regarding effectiveness of these drug therapy regimens and provides information to management, as required.
3. Educates pharmacy, nursing, medical staff and patients regarding safe and effective use of medication therapy.
- Assists in provision of clinical services.
- Documents education provided to pharmacy, nursing, medical staff and patients.
4. Participates, contributes and performs quality improvement initiatives as directed by the needs of the department.
5. Performs all duties of a licensed registered Pharmacist as required by management and as identified by the ability of the department to effectively provide comprehensive service. Maintains compliance with established procedures, policies, and laws in performing duties of a licensed, registered staff pharmacist.
6. Performs related duties, as required.

Job Qualification
- Bachelor�s Degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited college or university, required AND a minimum of two (2) years related experience including medication usage evaluation and distribution in a hospital, required - OR -
- PharmD AND a minimum of one (1) year PGY residency (or a minimum of two (2) years related experience) including medication usage evaluation and distribution in a hospital, required.
- Advanced certifications such as BCPS (board certification pharmacotherapy specialist), MHA, MBA, or MPH, preferred.
- Current license to practice Pharmacy in New York State, required.
