CNA / HHA Patient Escort
Northern Riverview Healthcare Center iis actively seeking a CNA/HHA to serve as Patient Escorts for our Skilled Nursing Facility.

DUTIES:
- Escort residents to and from appointments
- Observing and monitoring Residents.
- Reporting any health issues to the supervising nurse.
- Maintain confidentiality of all Resident & Facility data.
REQUIREMENTS:
- Valid New York State CNA/HHA Certification.
- Occasional heavy lifting may be required.
- Ability to follow the Plan of Care precisely!
- Must be able to work as a team member
- Valid photo id
