POSITION OVERVIEW: Under the supervision of the Assistant Program Director, Community Benefits Specialist will provide administrative support to case management staff as well as to the social work staff. This position will assist client in connecting to community resources such as, medical, mental healthcare and substance use disorder programming. This includes but is not limited to, record keeping, escorting clients to appointment as needed, completion of referrals, assisting in client programming and workshops, as well as maintenance of a reduced client caseload. Staff will also be required to client care/crisis intervention, administration of emergency first aid/CPR assistance and opiate overdose prevention intervention; related duties as assigned.

KEY ESSENTIAL FUNCTIONS: · Ensure high quality service delivery; including: conducting outreach efforts of new clients, conducting client meetings on time, coordinating services, and assisting clients with community resources.

· Develop and maintain active relationships with community organizations and providers (i.e. clinics, clubhouses, entitlement agencies, etc…)

· Escort client(s) requiring assistance to complete benefits applications and engage in services

· Assist Social Worker with planning and implementation of staff and client presentations.

· Utilize crisis prevention and intervention skills when needed.

· Maintain working knowledge of all technological systems utilized by the program and its affiliates.

· Meet all training standards.

· Complete, review and ensure timely submission of all internal and external reports.

· Complete all client benefits screens within 48 hours of client entry into the facility.

· Assist in preparation for housing applications; this includes but is not limited to the gathering of documentation.

· Utilize CAPs as a tool to direct work outputs.

REQUIREMENTS:

1) HS Diploma/Equivalent

2) CPR & First Aid

3) Overdose Prevention Rescue Training

4) Mental Health First Aid

5) CARES (Getting Started, Service Planning)

6) Motivational Interviewing

7) Basic computer skills (MS Office)

Job Type: Full-time

Pay: $36,000.00 per year
