Job Summary:
The Community Health Coordinator manages the entire administration of the behavioral health program including the management of the five enabling support staff and services that support the behavioral health program. This role is essential in managing and coordinating all referrals internal and external into our behavioral health program. This person will oversee federal, state and local reporting. This person will be responsible for creating and managing the budget portions of behavioral health, outreach, marketing and public health sections of the budget. This person works in support of the Psychiatrist, Clinical Therapists, and Primary Care Physicians/team to facilitate coordination and administration of referrals of patients with behavioral health problems appropriately to and from the division. This person will work alongside and support over 15 medical providers and 5-10 outside contracting providers. Supports various activities of the practice, tracking treatment plans and monitoring patient progress in conjunction with the practitioners of the department.
Frequently checks and maintains electronic medical records, electronic and phone communications. Screens and answers calls in a timely fashion. Ability to transcribe interaction with patients in the patient record. Initiates medication prior authorizations under the direction of the physician and completes when able by calling or faxing the information requested by insurance for approval, which includes completing the prior authorization form or answering questions regarding medical information via phone. Interfaces with patients, primary care providers, psychiatrist, therapists, administrative and support staff to foster efficiency and effectiveness of the health center. Maintains the schedules of the practitioners in the department including scheduling patients, reminder calls, schedule adjustment/blocking, and keeping an updated work-in list. Search and print TN Drug Monitoring Database records for each patient on a controlled substance prior to their appointment. Initiate letters of appeal, patient needs, and others upon the request of the provider. Monitors Suboxone patients� adherence to treatment plans and call patients/follow-up as needed to assure compliance with regulations.
Completes operational requirements by scheduling, referring, and following up on work results. Keeps providers informed by reviewing and analyzing special reports, summarizing information, and identifying trends. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, and establishing personal networks. Prepares monthly progress reports regarding schedules, analyzes variances, and initiates corrective actions.
Responsible for accurate and timely registration of behavioral health patients to include obtaining and verifying third party payer information, required authorization, referral and pre-authorization numbers, obtain/witness signatures authorizing treatment, and billing charges. Must have an understanding of implementation of the sliding fee scale. Maintains office services by organizing office operations and procedures, reviewing and approving supply requisitions and monitoring clerical functions
Maintains a pleasant and supportive attitude that fosters the therapeutic relationship between patients and the departments. Serves as a liaison between patients and the providers while creating a safe, comfortable environment. Contributes to team by accomplishing related results as needed. Maintains strict confidentiality guidelines for behavioral health related information. Demonstrates sound work ethic and maintains standard of conduct. Must have ability to plan time effectively, balance multiple tasks, work within stringent time frames, and execute projects to completion. Resolves problems, completes audits, identifies patient service trends, determines system improvements, and implements change. Must be flexible, self-motivated, and adapt to a changing environment.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices' patient population. Participates in the practice's quality improvement process and performance evaluation.

Required:
Bachelor Degree
Preferred:
Masters and/or Advanced Degree or Training Comparable with advanced responsibilities.
