The Opportunity:
This Work Matters!
The On-Call Community Health Specialist 2 provides tuberculosis (TB) support and health education services to individuals in their homes and in the community. This position creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community.
As the On-Call Community Health Specialist 2, you will:

Visit TB clients at their home or other designated place to observe them taking their tuberculosis treatment

Provide information to clients for needed services and support, and provide education about tuberculosis to the client and their family

Pick up or drop off specimen collection kits or other materials to clients as needed
-
Document services in Epic, including medication observation charting
-
Provide weight checks, vision screenings; coordinate specimen collection and deliver nutritional supplements and food to clients.

Plan and deliver interactive health education classes and presentations for a variety of groups; share culturally appropriate, department-approved health education and information with individuals, families and groups

Provide education that assists patients in identifying goals for behavior change

Minimum Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.

Education/Training: Equivalent to an associate's degree, AND
-
Experience: One (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

Other: Must have a valid state issued Driver’s License and BLS/CPR certification.
Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates. Previous experience working with vulnerable and diverse populations

Previous experience using EPIC

Experience working as a Community Health Worker, either in a paid or volunteer role

Strong customer service skills

COVID-19 Vaccination Requirement: To protect the health of the community and employees, Multnomah County requires employees to be fully vaccinated against COVID-19 or have an approved medical or religious exception as a qualification of employment.
