Community Health Worker will work closely with a licensed research nurse (RN) to implement a telehealth intervention with patients that have previously been hospitalized with COVID-19 as well as their Carepartners for a research study funded by the National Institutes of Health (NIH) National Institutes of Nursing Research (NINR) that seeks to improve current and future chronic illness management among adult African American COVID-19 survivors and their caregivers (Carepartners) in medically underserved and rural areas of South Carolina.

Maintain a calendar, schedule participants, conduct and document virtual/remote study visits with assigned research participants via telehealth.

Work collaboratively and efficiently with the research nurse to deliver the intervention component of the study per the protocol, study procedures, and in strict accordance with established timelines.

Works individually and in concert with COVID-Survivor and their respective Care Partner to operationalize the devised wellness goals/plans by providing resource navigation; facilitating access to care and other community resources in conjunction with identified needs and barriers.

Ensures accurate and timely data entry into the electronic research study database for study participants, while making sure policy, procedures, and guidelines are followed in accordance with UofSC/IRB and federal guidelines.

Attends team meetings, provides weekly updates, and progress reports, documents and follows participant adverse events through to resolution.

Other duties as assigned.

Minimum Qualifications

Bachelor’s degree in a related field and 1 year related research experience, or high school diploma and 5 years related research experience. Valid SC Driver’s license. Flexible schedule to be pre-arranged with supervising team leader. Some evenings/weekends may be necessary to meet project deliverables. Must obtain University of Miami (UM) CITI and GCP course certification within 14 days of employment.

Preferred Qualifications

Bachelor’s degree and one year experience in public health or non-profit administration, health administrative services or other health professional services or a high school diploma and 5 years related experience. Health care related fields or community organizing/advocacy experience related to meeting the health needs of underserved populations.

Knowledge/Skills/Abilities

Experience working with under-resourced communities or historically marginalized adult populations with comorbid health conditions. Experience with collaborative community based rural health research or clinical trials. Experience with Microsoft Office applications such as Word-Excel PowerPoint-Outlook-Teams. Excellent interpersonal and communication skills. Ability to multi-task a variety of functions. Organized, self-motivated, and detailed oriented. Must be able to work with limited supervision.
