Preferred Family Healthcare (PFH), Kansas City is seeking a part time Community Nurse with the passion and skills to deliver high quality healthcare. PFH has been providing primary health care to residents of the greater Kansas City area since 1984. Today you will join a healthcare organization and a staff of nearly 200 Direct Support Staff, Primary Staff, Program Coordinators, and other support staff to care for 60 clients. You will be welcomed into our diverse and caring environment to serve the community in which we live.

We have high expectations for you as one of our nurses. We need you to:

• Function as a key member of the Community Services team, often in a leadership role
• Conduct monthly face to face visits of each residential client in their home.
• Serve as a mentor to team members
• Complete an assessment/evaluation at the time of the visit and document findings on the Residential RN Monthly Health Summary.
• Ensuring needs for staff supervision/delegation or specialized instruction are conducted and documented on the designation form
• Conduct routine L1MA training and recertification classes
• Maintain communication with agency management and the client's support team including reporting and documenting all changes in client's health status and needs.
• Give critical feedback and insights to maintain the health and safety needs of our clients and training of our Direct Support Staff
