The Community Referral Specialist plays a meaningful role to our patients by working directly with our community physicians, and clinics to coordinate total patient care within the timeframe that the referring physician and the specialist feel are necessary. They will partner with patients and families to schedule post-hospitalization discharge follow-up appointments, tests, and procedures as prescribed by patient’s care team. Community Referral Specialists are the central hub for coordinating outpatient care needs post-hospitalization.
Community Referral Specialist Overview
Key responsibilities include but are not limited to:
- Answers all incoming calls from multiple sources and coordinates patient follow-up appointment plans.
- Serves as initial contact and coordinator for International patients, specialty care service line and other specialty clinics.
- Coordinates all necessary appointments and follow-up procedures while focusing on excellent patient relations and ensuring compliance with physician discharge orders.
- Improves the use of UMMC/UMP physicians as referral sources.
- Coordinates care schedule in a way that ensures patients receive timely patient care and is sensitive to the need for multiple return visits.
Minimum Qualifications:
- Some post high school education in service or medical access related area.
- 1+ year experience of clinic scheduling experience
- Robust understanding of current electronic medical records and scheduling systems.
- Proven phone management and organizational skills.
Preferred Qualifications:
- EPIC experience
Location: At this time, this position will be working remote. (MN residents only)
Hours: Monday-Friday 1:00pm-5:00pm
