What we offer:
The ConnectATX program supports families by connecting them to vital community resources through a multilingual helpline, an online resource tool, and our community-based organization partners. ConnectATX is committed to providing up-to-date information on critical resources and building partnerships with trusted community organizations to respond to our community’s needs. The Community Health Worker (CHW) position will be in the community and settings where individuals receive services and live. CHW will assess individuals using a Social Determinants of Health assessment and help them navigate and access resources that address their current need(s) and promote a healthy lifestyle. CHW will coordinate and implement community events such as health fairs and other health education events. CHW will work with other community health workers internally and externally to share best practices and strengthen education and outreach. CHW will also cross-train community partner staff on the use of ConnectATX. CHW will also participate in the Model Community Navigation Workgroup to inform operational and strategic direction.

What you’ll bring:
  • High school diploma or GED equivalent preferred
  • Experience working with unhoused persons, veterans, and immigrants/refugees preferred
  • Experience working with persons with disabilities and limited literacy skills preferred
  • Community Health Worker (CHW) certification or ability to obtain within 16 weeks of hire date
  • Understanding of the community through shared experiences and a strong desire to help people in vulnerable communities
  • Ability to communicate fluently in English and proficiency in another language (Spanish preferred)
  • Passion for connecting people to resources and removing barriers to accessing said resources
  • Access to safe transportation and the ability to travel within the Austin area
  • A natural curiosity paired with the ability to learn quickly about work that is new to you.

This position is located in Austin, Texas, and reports directly to the ConnectATX Navigation Manager, Cecilia Torre.
This position will be based in the community, working with partners at several locations and will be asked to report to the United Way office once per month for a regular check-in with the supervisor and training.
Key Accountabilities
Key accountabilities are likely to shift over time. Some areas will require more concentrated time in the initial stages as the person in this role develops the systems/processes.
Health Education and Social Care Navigation (70%)


  • Serve as a client advocate and liaison
  • Assess individuals utilizing social determinants of health survey
  • Conduct care coordination with individuals and connect them with relevant community resources
  • Assist individuals with completing applications for services
  • Educate and coach individuals to understand disease prevention and achieve positive health outcomes
  • Communicate client empowerment in navigating and interacting with health
  • Conduct follow-ups with individuals to understand referral outcomes and readdress any unresolved needs


Community Outreach and Training (25%)


  • Coordinate and participate in community events
  • Provide cross-training to community partners and their staff
  • Work with other community health workers to share best practices, strengthen education and outreach
  • Participate in Model Community Workgroups to inform larger care coordination efforts


Other Duties as Assigned (5%)


  • Support United Way’s collective efforts to fight poverty in Central Texas
  • Participate in appropriate organizational meetings
