Summary/Objective:

The CSS Counselor works in a 15 to 30-day short term clinical stabilization service as part of a multidisciplinary team. The counselor maintains a caseload of 4-6 clients and provides evidence-based individual and group counseling, psychoeducational services, as well as care coordination to residents who are COVID positive and COVID negative. The Counselor conducts assessments, treatment and discharge planning and collaborates closely with other members of the clinical team, as well as community providers.

Essential Functions

1. Facilitates therapy and psychoeducational groups, including identifying and implementing evidence-based practices.

2. Maintains an assigned caseload providing individual counseling multiple times per week to each client.

3. Provides case management services and works in collaboration with the case manager to execute referrals and linkages to the appropriate level of care.

4. Provides crisis intervention and risk assessments and screenings.

5. Provides coordination of care with all of clients’ providers, including primary care and outpatient services.

6. Supervises student interns, as assigned.

7. Completes all required documentation (biopsychosocial assessment, treatment plan, discharge summary, insurance forms, etc.)

8. Completes progress notes for all individual and group services provided to clients.

9. Attends and participates in staff meetings (daily and clinical). Attends trainings as assigned.

10. Participates in individual clinical supervision and accumulates hours toward licensure, if applicable.

11. Other duties as assigned.

Physical Demands

Duties may include frequent lifting. Will require sitting, standing and walking for extended periods of time.

Required Education and Experience

1. Masters in Social work or Counseling required.

2. Intermediate to advanced computer skills required (Word, Excel, PowerPoint, Access).

3. Valid driver’s license and ability to drive a company vehicle required.
