Department: COMCAREPay: $24.15 per hour | This position may also be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions.

Collaborate with both inpatient and outpatient treatment team to ensure the client admitted to Osawatomie State Hospital receives the best possible inpatient and outpatient psychiatric treatment. Plan and coordinate discharge of Sedgwick County residents from the inpatient setting at Osawatomie State Hospital (OSH) back to the community. Ensures that all patients that are released from the state hospital engage fully in outpatient services post discharge by engaging with consumers frequently during the transition to the community.
Direct Client Service
Create plans to ensure clients get to their mental health appointments
Confer with client caregivers, families, physicians, lawyers, community service providers and other professionals throughout the County and to secure and coordinate services
Assist clients to maintain basic needs (housing, food, medical)
Provide outreach, screening and engagement services to potential participants
Respond to the immediate needs of referrals
Arrange for the discharge of inpatient referrals and coordination of transition to the community
Facilitate Discharges from Inpatient Setting
Assist in implementing discharge plans for clients in the state hospital.
Set appropriate appointments for medical follow-up for patients with medical issues.
Provide frequent contact with clients for 3-4 weeks post discharge from the state hospital.
Ensure that individuals are engaged with outpatient providers post discharge.
Prepare written discharge plans to COMCARE staff, affiliate agencies and the state hospital prior to a client being discharged.
Monitor the implementation of discharge plans to ensure the clients have all recommended services promptly on their discharge from the state hospital.
Act as a liaison between all the COMCARE programs and affiliates regarding consumers that are inpatient at the state hospital.
Travel to the state hospital on a weekly basis to engage with individuals prior to discharge.
Treatment Planning and Documentation
Prepare Treatment Plan including client and family participation on cases where needed.
Collaborate in review, updates and progress on goals with Treatment Plan coordinator or other members of the treatment team.
Complete progress notes in accordance with COMCARE guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within prescribed timelines.
Complete Notes for Record, Precautions statements and authorizations for release of records as required and/or recommended.
Enters no show and cancellations for accuracy and completeness of medical record.
Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE. In order to comply with the requirements of the federal health care staff COVID-19 vaccination mandate, all employees of COMCARE and the Health Department must be fully vaccinated.
