JOB SUMMARY

The Direct Care Staff (DCS) under the direction of the Qualified Intellectual Disabilities Professional (QIDP), Registered Nurse (RN) and/or Licensed Vocational Nurse (LVN) provides direct and indirect client care activities. The DCS assists clients with activities of daily living, provides personal care, motivates and assists clients in developing and utilizing their maximum potential for self-care, comfort and assists in the maintenance of a safe and clean environment for an assigned group of clients.

The DCS duties include assisting clients with bathing, dressing, feeding, toileting, repositioning in bed, walking, social activities; obtaining vital signs, height and weight; transports clients using wheelchairs and/or movable beds in consideration of client age, gender, culture and stated preferences, maintaining client privacy and respect at all times. The DCS assists with admission, transfer, or discharge activities as directed by QIDP, RN and/or LVN.

SPECIFIC FUNCTIONAL RESPONSIBILITY:

Activities of Daily Living

  • Assists with bathing and dressing
  • Assists in bowel and bladder activities such as use of bedpans, urinals, or commodes
  • Keeps incontinent clients clean
  • Assists with personal and dental hygiene
  • Ensures clients are dressed properly
  • Provides general nursing care such as proper positioning, lifting and turning, and ambulating the clients
  • Prepares clients for meals; serves and removes food trays and assists with feeding clients, if applicable
  • Engages clients with a positive and friendly demeanor, while providing support through social interaction at scheduled and unscheduled activities

Procedures, Documentation, and Reporting:

  • Measures and records vital signs (temperature, pulse, respiration, blood pressure, and oxygen saturation)
  • Measures and records height and weight
  • Monitors and records bowel and bladder as directed by QIDP, RN and/or LVN
  • Provides and discards bedpans, urinals, and emesis basins
  • Recognizes and reports changes in client’s condition to nurse
  • Ensures that clients’ skin is kept clean and dry and reports reddened areas or skin breakdown to nurse
  • Provides information to oncoming shift to ensure continuity of care
  • Participates in and contributes to interdisciplinary care conferences

Client Care Area

  • Lifts and turns with proper and safe body mechanics
  • Follows and ensures infection control policies and guidelines are followed for assigned clients/rooms.
  • Transports and accompanies clients to and from various departments, as directed
  • Maintains neatness and order in the client unit, including making the bed and necessary linen changes. Disposes of trash and linen properly
  • Assists in placing and removing protective devices such as bedside rails as ordered

Support to Licensed Nursing Staff

  • Assists nurse with admitting, transferring, and discharging clients
  • Receives and accurately inventories and secures client belongings as appropriate
  • Assists nurse in giving treatments and examining clients
  • Assists with restorative and rehabilitation care with proper instruction and supervision
  • Notifies appropriate licensed personnel when client complains of pain or pain is suspected
  • Answers call bells and makes rounds on assigned clients

REQUIREMENTS:

  • Current Certified Nursing Assistant certification issued by the California Department of Public Health preferred
  • Current American Heart Association Basic Life Support (BLS) certification required
  • Minimum of one year of DCS experience in intermediate care facility preferred
  • Able to lift thirty (30) pounds without assistance
  • Must be able to read client information, posted materials and other printed material
  • Knowledge of basic nursing procedures and charting as applicable to job
  • Must possess good work ethics and a professional image at all times
  • Knowledge of CMS conditions of participation, CDPH state regulations, CCS, and TJC (hospital, nursing care center, and laboratory) accreditation requirements as applicable to job
  • Working knowledge of computer and software applications as applicable to job
