The Discharge Specialist meets with Delaware County individuals to effectively facilitate the discharge process from Crozer`s Acute Mental Health Inpatient Unit. The goal is to increase ambulatory follow up after discharge as well as address any barriers that would affect successful follow up after hospitalization. As the primary discharge facilitator and educator, the Discharge Specialist ensures mental health consumers have access and connection to all services that are necessary for tenure and progress in the community. Performed duties are recovery focused, strength based, and trauma-informed in support of the Medical Center mission to ensure the highest quality of client care in an economically sound and efficient manner.
This position serves: Adult consumers 18 years of age and up.

  • Establishes rapport with individuals while individuals are on the unit prior to discharge from Crozer`s Mental Health Acute Inpatient Unit.
  • Meets with individuals upon discharge from the unit.
  • Reviews Project Red Indicators/Components and follows up with responsibilities.
  • Reviews Social Determinants of Health indicators and makes referrals as appropriate.
  • Collaborates with inpatient staff and outside providers in an effective manner.
  • Determines individual`s need for transportation to their ambulatory follow up appointments; set up transportation if needed.
  • Obtains consents for contacting outside providers including Delaware County Satisfaction Team (Voice and Vision) and sends to OBH
  • Appointment reminders the day before for follow up appointment.
  • Discusses Certified Peer Specialist Services and refers if interested in CPS Services.
  • Offers reminder calls for follow-up appointments and remains involved until individuals are successfully connected to follow-up providers.
  • Willingly and readily, assists all internal and external customers as needed, looking beyond individual scope of service.
  • Interacts with patients, visitors and other customers in a courteous, professional and service oriented manner.
  • Communicates in a positive, helpful and friendly manner at all times, using appropriate body language, eye contact and tone of voice in person, electronic communication, or by telephone.
  • Assists with care and maintenance of equipment and supplies.
  • Maintains and orderliness and cleanliness of work area.
Openly communicates problems, concerns, and/or suggestions in a constructive, solution-focused, and professional manner and in an appropriate forum.
  • Confers with the treatment team and appropriate members of the client's network in order to coordinate care as evidenced in progress notes.
  • Encourages community and natural support networking.
  • Tracks data and outcomes including member tracking, quarterly breakdown, and percentage engaged.
For high priority and urgent cases will act as Engagement Specialist and engage clients for BCM Services
  • File paperwork
  • Adheres to departmental polices and procedures.
  • Attends to annual mandatory in-service programs and maintains current knowledge of safety practices with established procedures.
  • Maintains knowledge of current trends in the behavioral health field and is open to new learning opportunities.
Utilizes sound judgment, seeks supervision when appropriate, and follows safety protocols.
  • Operates as a self-starter in developing new ideas, making suggestions and actively pursuing increased responsibilities, as evidenced in supervision.
  • Understands confidentiality of records and consumer information. Complies with all areas of HIPPA.
