JOB SUMMARY: The DMI Quality Analyst is responsible for converting paper medical record documents into digital images as well as ensuring the timeliness, accuracy, and accessibility of the digital images in the electronic medical record. The DMI Quality Analyst completes analysis of all paper documents, determining which forms are maintained as a permanent part of the medical record and which are discarded or forwarded to other areas for use. The DMI Quality Analyst utilizing scanning equipment to convert the paper documents to digital images for uploading to the electronic medical record. Knowledge of the equipment and programmed used to convert images is crucial to ensure high quality imaging. This is particularly important with color and finite images such as EKG and other types of tracings. The DMI Quality Analyst is responsible for all inspection, determination and proper notation of poor quality or missing images imported into the electronical medical record. The DMI Quality Analyst is also responsible for proper alignment and rotation of permanently stored images, in addition to proper patient labeling on each image, ensuring no other patient is listed within another patient’s record. Specific job duties include but are not limited to: gathering, organizing, and verifying all applicable hospital medical record documents post-discharge, completing loose document processes to ensure each document reaches the patient’s electronic medical record to which they belong, ensuring patient identification on every page and that every page belongs to the correct patient, prepping each chart with correct encounter pages and removing any items which will interfere with imaging, and imaging all applicable materials into the electronic medical record AND verifying the quality of all images, taking appropriate action on any image errors, identifying trends in imaging errors and processes, grouping, relocating, and rearranging documents properly within the electronic medical record, researching batch errors and verifying the readiness of the batch for the validation process, and assisting in quality assurance review of imaged and indexed records. Recognizing new forms and communicating to ensure new forms are properly routed to supervisor for awareness in the event of barcoding or other measures that may need to be taken for a new form. Importing History and Physical reports, office records or other pre-surgical documents into the patient’s electronic medical record as directed. Performs other duties as assigned.
QUALIFICATIONS:
Education: High School graduate or equivalent required; Associates Degree required; Medical Office Quality Analyst or Technician certificate or RHIT/RHIT-eligible preferred.
Experience: Minimum 1 year experience with medical records and PC use in a hospital or physician office setting preferred.
Auditing, QA review, process improvement, or similar experience in any field preferred.
Experience in document imaging strongly preferred.
Other: Excellent and positive interpersonal, verbal and written communication and presentation skills;
Able to work with other care team members and leaders as a team to reach facility and organizational goals;
Proficient with basic office equipment, including scanner, PC, printer, copier;
Must be able to round at the Valdese campus and Morganton campus;
Must able to think critically and creatively to positively problem solve;
Must be able to use LEAN principles and tools to improve processes;
Must complete tasks with integrity, accuracy, consistency, and compliance;
TYPICAL PHYSICAL DEMANDS: Corrected vision and hearing to normal range and ability to discern color; eye-hand coordination and manual dexterity. Full range of body motion including bending, stooping, kneeling, standing, and sitting for prolonged periods of time; lift, carry, push or pull objects weighing up to 15 pounds; repetitive motion.
