Statewide positions available! NFI North’s Care Management Entity has openings for TrECC Care Coordinators to assist youth and families in the new Transitional Enhanced Care Coordination (TrECC) program. This is a hybrid position working from a home office and in collaboration with the CME Administration offices located in Tilton and Littleton NH. We’re looking self-starters with great organizational skills and a strong work ethic. Some travel throughout the state is required. All aspects of TrECC are designed to reflect System of Care values including family voice and choice, cultural and linguistic competency, trauma informed practice and unconditional care for the youth and families served. The TrECC program utilizes ongoing collaborative analysis and implementation science to improve practice and ensure best outcomes for families as the program continues to develop and grow.

This position includes duties such as:

Coordinating Plan of Care facilitation, completing assessments and providing care coordination as required. TrECC Care Coordinators will be participating in individual care planning, identify and engage natural supports, participate in all meetings, supervision and professional development activities as required. TrECC Care Coordinators will facilitate intake process for new referrals, maintain ongoing contact with participants and their families in their home, residential treatment episodes and community engagements as indicated and required. Facilitate team and other meetings as defined by each participant’s Plan of Care. Provide support for family members as required and indicated by service needs. Work with families and identified participant in creating a crisis safety plan. Advocate for participant's and their family’s needs as appropriate. Coordinate the transition of services as needed. Maintain documentation of treatment and services including writing of court reports for each assigned participant. Available for crisis support and participation in an on-call system as needed. Perform all duties in accordance with the agency’s policies and procedures. Ensures compliance with Joint Commission, federal, state and local regulations and standards. Travel is a key component of this position.

The candidate would be detail oriented, flexible and able to multitask, have excellent verbal, written, and computer skills to include but not limited to expertise in Microsoft Word, Microsoft Excel, and experience with electronic systems (E.g.: HRIS, EHR) and be able to collaborate well with outside resources.

Qualifications: A Master’s degree is preferred and 2 years’ experience with high risk populations. Salaried position; $50,000 annually.
