The primary purpose of the position is to provide oversight and organizational management of the Environment of Care to ensure a safe, functional, supportive, and effective environment for residents, staff members, and other individuals in the facility. The position is crucial to the mission of Longleaf in ensuring that quality care is provided, good outcomes are achieved in an environment of care that is safe, and compliance with regulatory agencies such as CMS, Division of Public Health, SSOHF, DHHS are maintained.

This position is the Coordinator of the Facility’s Environment of Care Program. This program comprises seven functional plans. These plans and their respective function leaders as outlined:


  • Safety Management, Safety Director
  • Security Management, Police Chief
  • Hazardous Materials and Waste Management, Joint leadership by Plant Operations Director and Infection Control Nurse
  • Emergency Preparedness (Disaster) Management, EOC Coordinator
  • Fire Safety Management, Joint leadership between Safety Director & Plant Operations Director
  • Medical Equipment Management, Plant Operations Director
  • Utility Systems Management, Plant Operations Director
  • Legionella Remediation and Water management, EOC Coordinator

  • Ensures regulatory compliance of CMS standards and other regularity agencies as it pertains to orientation and training, coordination of facility inspections; and the coordination of facility-wide, departmental, and some job and task-specific policy and procedures.
  • Oversees the development and implementation of plans to meet these standards.
  • Monitor, evaluate and develop aggregate data and information streams that are necessary to measure safe (and unsafe) performance in the facility’s environment of care, and to identify opportunities to improve.
  • Identify, collect, and aggregate data about problems, failures, staff errors and behaviors. Identify trends, patterns, and underlying causes of injury, occupational illness, and system failures (per shift, supervisor, etc.).
  • Identify key issues for the EOC Committee focus based on the potential severity and frequency, as well as the potential to improve. Prioritizing and setting timetables to correct deficiencies.
  • Set goals and develop strategies, analyzing data, addressing problems, and coordinating plans.
  • Apply Performance Improvement principles to identified key problems. For example, if safety has a deficiency in evacuation plan or plant maintenance needs to address a lighting problem in a resident area, this position will help prepare action plans to address problem as it relates to regulations. (The actual responsibility for carrying out the plan rests with the function leader.)

Minimum Education and Experience Requirements

Master's degree in psychology, social work, education, health, business administration or related human services field and one year of experience in professional level treatment programming for the applicable client population
OR
A bachelor' s degree in one of the fields listed above and two years of experience as indicated above
OR
An equivalent combination of education and experience. .
