Extended Care Coordinator - Overnight

About the Position:

The role of the Extended Care Coordinator is to provide a welcoming, supportive and structured community living experience which meets or exceeds client expectations for service during Extended Care.

Schedule:

Thursday-Monday 12:00am-8:30am

Your Role

• Facilitate 12 step support groups
• Facilitate life skills related groups and outings
• Perform baggage and property searches according to established guidelines
• Coordinate transportation for outside appointments, pickups and drop offs for new admissions, current clients and discharging clients as assigned. Provide transportation as needed
• Immediately communicate to Manager and Counselors any behavioral problems or safety concerns which may arise when dealing with clients
• Assist with morning wake-up and evening bed check of clients as necessary
• Perform routine inspections of client rooms, reporting any infractions of ECare resident guidelines
• Assist with store run purchases on behalf of clients providing transportation to stores on designated nights.
• Facilitate ordering and pick up of prescriptions for clients. Facilitate program groups and other activities as directed/needed.
• Ensure clients complete scheduled client surveys
• Offers real-time mentorship and guidance to residents living in the house
• Monitors sign in/sign out sheet and record management to ensure all extended care residents are accounted for
• Ensures all residents are attending the required amount of AA and NA meetings
• Ensures house is properly stocked with supplies, linens, comforters etc.
• Provide transportation to Clinical, Meals and local 12 step meetings.

Qualifications:

• High School Diploma or Equivalent
• Experience/knowledge in substance abuse treatment - preferred

Compensation:

The base rate of pay for this position is $15.00 to $22.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.
