Job description
- Provide hospitality to clients and ensure a safe and welcoming environment. Assist in completing initial intake for those who use the program. - Provide clients with an orientation to their program and explain services that Housing Forward offers. Provide on-site support to clients enrolled in the programs. - Use good communication/engagement techniques without any bias to build rapport with clients and create a “safe for all” and healthy environment. - Develop resources in the community accessing “The Hub” and other online or networking. - Distribute client’s transportation links to Dr. appointments or other necessary appointments where travel is needed - Learn Housing First, Motivational interviewing and Trauma informed care processes in our on-line learning modules. - Communicate and cooperate with staff from other programs as needed to ensure seamless delivery of service to clients. - Work as a team with the other RISE and Sojourner House case managers to alternate responsibilities and negotiate tasks to ensure safety and that all duties are fulfilled. - Collaborate with and support volunteers working in the programs. - Oversee client’s use of Community Center and assist in creating activities within to encourage clients to participate. Administrative: - Enter client intakes into the HMIS system safeguarding client’s confidentiality by observing guidelines for exchange of information. - Document all activity with clients into case notes in HMIS - Attend and participate in weekly RISE center case management meetings and quarterly team meetings, monthly trainings, and all other required meetings. - Be responsible for safety and proper use of any Housing Forward provided equipment. - Other duties as assigned by supervisor. Knowledge, Skills: Excellent engagement skills, ability to work independently and as part of a team. Knowledge of issues related to homelessness and poverty. Strong interpersonal and problem-solving skills.

Member Duties : The outreach and engagement specialist is responsible for 1) intake, assessment, and referral services to clients in the Emergency Shelter; 2) Assist in the facilitation of moving clients beyond shelter services by linking clients to basic services and case management team; and 3) provide crisis intervention, advocacy and education on behalf of clients. The outreach and engagement specialist strives to build a trusting relationship with shelter guests in an effort to encourage access to services and housing and reduce the length of stay in the Emergency Shelter
