Family case planner in a child welfare prevention program serving families with children under the age of 18 who are at risk of placement. Families in the Special Medical prevention program also have medically fragile children in addition to other risk factors.This is a home based program that serves families in all 5 boroughs of NYC by providing casework counseling and utilize the Solution Based Casework model to partner with families to agree upon the concern and behavior that needs to change to ensure children remain safe at home.
MSW or equivalent degree preferred. Knowledge of child welfare and/or similar experience working with children and families.
