The Family Partner provides support to youth and families served by the program and assists them to successfully engage with and navigate the Native American Rehabilitation Association’s (NARA’s) wraparound services and help reduce barriers to access. The position facilitates assessment of services, makes referrals to services as appropriate for the client’s needs, and follow-up on referrals to determine outcome of interventions. The position ensures services are family driven and child focused, will reach out to youth and families, orient families to wraparound services, coordinate and participate in child and family team meetings, work with other identified services providers to ensure services are strength-based, client-centered, and focused on meeting needs.

Job Duties

  • Familiarize families with the wraparound model and help them navigate the system and reduce barriers to accessing services.
  • Mentor clients by serving as a role model and providing support based on relevant knowledge and experience.
  • Carry case load and facilitate groups.
  • Facilitate assessment of services and make referrals to services as appropriate for the client’s needs.
  • Assist clients in completing applications and setting appointments with community resource agencies to facilitate access to transportation, housing, medical care, and other resources.
  • Establish and maintain progress notes in client files for all active cases.
  • Meet clients at primary care provider assessments or mental health assessments when appropriate.
  • Comply with all applicable federal and state laws, regulations, and guidelines as well as NARA’s policies and procedures regarding mandatory reporting, confidentiality, and any other applicable components.
  • Work with wraparound team as outlined in plan of care.
  • Attend all wraparound meetings.
  • Work with youth and families using appropriate interventions.
  • Participate in community mental health awareness and outreach programs for NARA.
  • Participate as a member of the treatment team, collaborating and consulting with other members to provide client care.
  • Implement strategies to inspire, encourage, motivate, and instruct clients through relapse prevention and recovery.
  • Introduce clients to appropriate community supports.
  • Seek and accept clinical and administrative supervision.
  • Act as a liaison with internal NARA program and services and outside community resources.
  • Assures work in and with the community promotes and increases positive relationships with NARA, its programs, and staff.
  • Other duties as assigned.

Qualifications

  • High school diploma or GED required, Bachelor’s Degree preferred.
  • QMHA Credential, or the ability to be credentialed as a QMHA
  • At least one year of related experience.
  • Must have personal and/or professional experience assisting caregivers with children/adolescents who are receiving mental health services required.
  • Successful experience working with community resource and social services agencies preferred.
  • Ability to organize and maintain activity logs, contact tracking forms, and other documentation.
