• Integrated Behavioral Health (IBH) Services

Description of Role:

HHMC & FRC is seeking a Family Practice Physician that will see patients independently, in collaboration with mid-level providers as well as patients referred by other Physicians. A major purpose of this role will be to assist in increasing patient encounters per year and to participate in the development of the integrated healthcare model.

Essential Functions:

Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment for medical conditions, and any problem detected.

• Manage acute and chronic illnesses, develop/coordinate a plan of care, and determine the need for case conference.
• Make appropriate referrals to both the internal disciplines of the clinic and/or external agencies for additional services required.
• Work with the Behavioral Health Specialist to provide continuity of care for the mental health population.
• Provide clinic consultation to mid-level providers as needed.
• Provide accurate, timely documentation in the patient’s medical record of patient contacts, case planning and plan of care.

Qualifications:

• Current California Medical License (Required)
• Clean, Current DEA Record (Required)
• COVID-19 Vaccination (Required)
• BC/BE (Preferred)
• Overall knowledge of medical office patient care
• Knowledge of confidentiality issues involved with release of medical information
• Must have excellent interpersonal skills including ability to interact with staff and providers at other medical facilities

Job Type: Full-time

Pay: $239,886.00 - $254,301.00 per year
