Family Resource and Outreach Coordinator

Annual Salary Range: $61,568. - $73,465.

Position Summary: The Family Resource and Outreach Coordinator will work with correctional staff to respond to inquiries, requests, concerns and/or complaints raised by family members of individuals in the custody of the Community Reintegration Center (CRC). The coordinator will also engage families through outreach and education and provide information, assistance, referrals, and support to family members during the incarceration period.

Position Duties:
  • Working with staff and mental health experts to identify residents who may be experiencing distress. Work to educate residents family members about rules and policies regarding visits, mail, and phone contact and other routine information. Conduct proactive outreach.
  • Create relationships with support services, community partners, hospitals, mental health clinics and inpatient units that would be referral sources. Assist residents' family members with their questions in a respectful and nurturing manner.
  • Provide meetings and complete tasks as assigned within roles of manager. Participate in committees throughout the year to assist with various projects, i.e., Care Coordination Appreciation, Resource Fairs and Wellness planning committees.
  • Maintain a database of activities and those contacted, along with final results.
  • Will be a family liaison for reentry planning meetings. Will conduct wellness checks on residents. Will be required to conduct emergency notifications to residents and to residents' families.
  • At all levels of program functioning and interactions, specifically in the realm of outreach activities. Move racial equity forward through equitable/inclusive use of an anti-racist, pro-equity, collaborative approach. Analyze situations with a racial equity lens.
Minimum Education & Years of Experience Required:
  • Must be a current resident of the State of Wisconsin.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Bachelor's Degree in Social Work, Human Services or Education.
  • At least one (1) years of recent experience working in Education or Human Services
Preferred: Graduate Degree in Social Work, Human Services or Education

Knowledge, Skills, and Abilities: Demonstrated interpersonal skills showing an ability and sensitivity to working with and engage a variety of people and stakeholders. Knowledge of community resources and the ability to utilize them effectively. Ability to be a creative self-starter (think out of the box ideas) with attention to detail, accuracy, and excellent organization skills. Ability to build and support community partnerships and outreach within diverse communities, excellent verbal and written communication skills including, proofing, grammar, spelling, text editing, and writing. Ability to communicate with the large and small groups including presentation skills. The ability to liaise with individuals from both public and private organizations.
