Job description
- Maintains and manages a caseload as assigned by the Housing Services Coordinator for those program participants that want Harm Reduction services.
- Facilitates one to one visits with each program participant on case load at least twice per month
- Develops Harm Reduction Service Plans with program participant that include but are not limited to identifying triggers, relapse prevention, linking with community sober supports, assists with linking clients to medication assisted treatment, incorporating coping skills, and linkages to mental health and primary care providers.
- Assists with, reviews, and monitors program participants linkages necessary to successful community integration
- Creates and maintains collaborative working relationships with other agencies to better serve program participant needs
- Facilitates Harm Reduction groups 5 days per week
- Demonstrates and encourages flexibility in response to changing needs of program participants and agency.
- Writes specific and concise individual and group Harm Reduction progress notes in RISE
- Conducts breathalyzer and toxicology screenings as necessary to use as a tool in Harm Reduction support
- Completes all required paperwork and documentation in a timely manner
- Maintains and updates program participant Harm Reduction chart
- Attend and/or facilitate weekly staff meetings and case conferences
- Attend regular community meetings as assigned by supervisor
- Attend trainings as assigned by supervisor
- Carries out additional duties essential to the above-stated responsibilities as assigned.
- Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and
interactions with fellow staff, program participants, and external partners.
- Maintains compliance with all required rules and regulations including Federal
Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.
Requirements:- Master’s degree in Human Services or related field and a minimum of 6 months’ human service experience. Field placement as an intern may be applied to the required human service experience. Or
- Bachelor’s degree in Human Services or a related field and a minimum of one year of human service experience. Field placement as an intern may be applied to required human service experience. Or
- Associate’s degree in Human Services or a related field and two years’ human services experience in the appropriate field. Field placement as an intern may be applied to required human service experience.
- Peer, CASAC, or other related certification and a minimum of one year of human service experience. Field placement as an intern may be applied to required human service experience.
- A valid New York State driver’s license is required
