Position Overview : : :
Syringe Access Services, a program of the San Francisco AIDS Foundation, is the largest syringe access program in the State of California serving over 13,500 participants per year. The HCV Coordinator is responsible for outreach and engagement with people who inject drugs (PWID) and engaging HCV+ PWIDs in Syringe Access Services’ Hepatitis C Wellness Program.
Engagement includes: : life stabilization/HCV treatment readiness coaching and enrollment; linkage to primary care, including a medical evaluation; support initiating and adhering to hepatitis C treatment. The Coordinator also provides harm reduction group and individual level interventions, overdose and HCV prevention education and programming. Responsible for client level data collection and documentation, developing collaborative relationships with health and social service providers to facilitate effective linkage, and data entry and reporting.

Skill in operating office equipment such as a personal computer, copy machine and telephone system. Ability to perform routine bending, stooping, twisting, and reaching. Ability to lift a minimum of 50 pounds. The position requires work during evenings, weekends and holidays.

This position is primarily located at 1035 Market Street, 117 6th street, and at satellite locations homeless encampments, and van-based services in the Bayview,: however may require traveling to other office locations or off-site meetings as required by the job.

- PWID Outreach and Engagement: :Engage and link clients to HCV antibody testing and confirmatory testing; recruit PWIDs to participate in SAS’s HCV Linkage and Wellness Program through relationship building, recruitment and retention activities; and develop outreach plans, outreach materials, and educational and resource flyers.
- HCV Prevention Education and Health Education: :Provide health education & referral to PWIDs at SAS HCV testing sites and through individual and group risk reduction sessions to prevent HCV transmission and prevent reinfection. Responsibilities include staging and breaking down/cleaning the 6th Street Harm Reduction Center & other sites, building ongoing relationships with people who inject drugs, providing on site referral and linkage to substance use, health and social services. Provide health coaching focused on self-care, health improvement activities, disease prevention, etc. Provide individual and group motivational interviewing sessions. Develop and facilitate workshops and health improvement, education, and skills building groups. Provide DOPE Project overdose prevention and response trainings, recruit participant volunteers for low threshold volunteer opportunities, and act as an ambassador for SFAF’s harm reduction programming in the 6th street harm reduction center and the community. Train, supervise, and retain volunteers. The 6th Street Harm Reduction Center welcomes diverse groups of people who are active substance users, are experiencing homelessness and/or marginally housed, have untreated mental health concerns, histories that often include trauma, incarceration and violence. SAS staff provides de-escalation, conflict resolution, and crisis interventions. Document and enter site data, referral data, volunteer data, DOPE Project data, HCV data, partner services
- Wellness Coaching and Treatment Preparation and Readiness Counseling:: PWID may face significant barriers to successfully engaging in medical care and adhering to HCV medications. Wellness/Treatment readiness coaching will include strategizing around substance use management, nutrition, acquiring identification, Medical, creating or strengthening routines and social connections, overdose prevention, linking to mental health counseling, and primary care.
- HCV Healthcare Navigation: :Assist PWIDs in navigating the medical system to link (or reconnect) with primary care, complete a medical evaluation, initiate hepatitis C treatment including making medical appointments, assisting with enrollment in primary care, medical care accompaniment, proving transportation assistance to and from medical care visits, and assisting PWIDs in navigating their doctor-patient relationship and communication.
- Harm Reduction and Substance Use Counseling:: Responsible for providing substance use and harm reduction counseling through motivational interviewing in groups and individual sessions. Assist PWIDs in contemplating change, making goals, and plans to meet them.
- HCV Medication Adherence Support: :Assist PWIDs with storing and adhering to prescribed HCV treatment medications and host adherence supporting groups and peer support building groups.
- Data Collection and Documentation:: Document and enter data on outreach, individual sessions, group interventions, medical visits, medication adherence and incentives.
Essential Qualifications : : :
- Knowledge of substance use and health concerns of injection drug users in an urban setting.
- Knowledge of health and social services and referrals throughout the city of San Francisco.
- Awareness and sensitivity to diverse and marginalized populations.
- Ability to speak candidly and knowledgeably about drug use and sexual risk.
- Ability to implement de-escalation techniques, follow safely protocols, and use sound judgment in making independent decisions.
- Ability to work in a fast-paced environment with people who are using substances and are experiencing homelessness.
- Proven ability to make independent and sound decisions based on good judgment.

Education and Experience : : :
- Three years’ experience working with drug users, highly marginalized, or homeless populations required.
- Associates Degree preferred
- Experience using motivational interviewing preferred
- Strong understanding of harm reduction practices and principles preferred
- Proof of valid California driver’s license with an excellent driving record and driver’s insurance required.
- Understanding of HIV/HCV disease prevention and treatment.
