Position Summary:

The Health & Wellbeing Navigator transforms the experience of aging at Orchard Cove through a proactive approach to health and wellbeing powered by the Vitalize 360 platform. This is accomplished by having meaningful one-on-one conversations with Residents and supporting them to articulate what matters most, to create a wellbeing plan for their life, and to educate them in making decisions that align with what matters most to them today. This person partners with teams and services throughout Orchard Cove in order to support each Resident’s life and health priorities through group programs and culture building.


The Health & Wellbeing Navigator is part of an interdisciplinary team serving as go-to experts on care, service and living options throughout Orchard Cove and beyond. Through interdisciplinary collaboration, the Health & Wellbeing Navigator helps to ensure that Residents at Orchard Cove are living within the level of care that aligns with their what matters most priorities.


The Health & Wellbeing Navigator is an integral part of empowering older adults to create a life full of meaning and purpose, thus transforming what it means to age well.

Position Responsibilities:

  • Manage and curate the wellbeing philosophy and process for ensuring every Resident at Orchard Cove is guided to articulating ‘what matters most’, is empowered to identify their life and health priorities, and is motivated to make decisions based upon these preferences.
  • Inspire the Orchard Cove community to embrace the philosophy and practice of Vitalize 360 as the platform of life through the development and facilitation of health and wellbeing education seminars.
  • Serve as the lead partner for Residents through the process of making decisions about care and services that are aligned with their what matters most preferences.
  • Part of the team who provides education, communication, and support to Residents and the people who matter most to them about options and resources at Orchard Cove and beyond.
  • Lead and partner with the interdisciplinary team to share important information about what matters most to Residents as it relates to achieving their life and health priorities. Lead the Navigation conversation during the admissions interview process for Independent Living prospects, and serve as a member of the admissions team setting criteria and making admissions decisions. Complete other projects as assigned.


Qualifications:

  • Coaching communication skills
  • Passion for transforming the experience of aging, and helping older adults live their best lives possible
  • Previous knowledge and experience working with older adults and their families
  • Ability to work independently and as part of an interdisciplinary teams
  • Flexibility to meet assigned work, and take on new challenges as they arise
  • Bachelor’s degree at a minimum strongly preferred


Working Conditions and Physical Demands:

  • Ability to move around the office area
  • Ability to occasionally move items up to 20 pounds
  • The individual spends over 95% of his/her time in an air-conditioned environment with varying exposure to noise. There is protection from weather conditions
